POSITION SUMMARY:

The Sickle Cell Social Worker will play an important role on our team. The SickleCell Social Worker has expertise in understanding the impact of sickle cell diseaseon patients and their families and ensures the delivery of high-quality services toindividuals living with Sickle Cell Disease and their families. Your work will includepsychosocial assessments, supportive counseling, brief interventions forbehavioral health, resource coordination and navigation, and advocacy support.There are additional opportunities for program development, research, andparticipation in a wide variety of activities.As an inherited disease, the patients have complex psychosocial needs whichstem from chronic medical conditions across their lifespan. In the United States, itis estimated that Sickle Cell Disease occurs among about 1 out of every 365 Blackor African-American births and among about 1 out of every 16,300 Hispanic-American births. Many patients experience depression or anxiety living with sicklecell disease.

The clinician will serve as a member of the multidisciplinary team and collaborateto ensure the behavioral health needs of the patients and families are met. Clinicalresponsibilities for services include but are not limited to, conducting psychosocialassessments, individual therapy, crisis interventions, leading groups anddevelopment of programs for parents, formulating psychosocial diagnoses andtreatment plans, and implementing effective, evidence-based treatments forpatients and family members. The clinician will also provide education/teachingregarding behavioral health conditions and treatment to providers and staff. As anintegral part of the multidisciplinary team, the sickle cell social worker willcomplete psychosocial assessments, provide supportive counseling, and work withfamilies to address psychosocial needs. The sickle cell social worker will have theopportunity to build relationships with families over time and to work with familiesimpacted by both acute and chronic illness. Primary focus: caregivers and theirchildren.

CORE DUTIES:

  1. Attendance at CWCs weekly multidisciplinary team meetings and educationprogram offerings;
  2. Attend continuing education training necessary to work toward licensure;
  3. Establish and maintain effective working relations with patients/clients;
  4. Evaluate case problems and provide leadership and consultation in thedevelopment and implementation of case plans;
  5. Understand, interpret, and apply regulations and departmental policies andprocedures;
  6. Plan, schedule, and conduct individual and group meetings;
  7. Provide clinical case management and therapeutic services.
  8. Interviews patient/patient's social support system to complete and initiatepsychosocial evaluation;
  9. Provides psychoeducational groups to support patients and the patient's socialsupport system.
  10. Provides ongoing assistance to patients/patients’ social support systems incoping with stress related to hospitalization, disability, and chronic/terminal illness.
  11. Works collaboratively with the patients and their social support system toestablish and implement a plan of care with a multidisciplinary team that meets thepatient's identified psychosocial needs.
  12. Conceptualize, develop, and implement a comprehensive system of careprogram to support patients and the patient's social support system (a system ofcare ranging from medical, psychosocial, dental, educational, legal, housing, food,spiritual, and financial stability).
  13. Engage with community stakeholders and agencies to build capacity andcollaboration.
  14. Develop and conceptualize evaluation feedback to determine the effectivenessof services
  15. Conduct a 3-5 year strategic plan.
  16. Conduct outreach to network with local, state, and national organizations thatare providing services for patients and patients’ support systems.

RESPONSIBILITIES, SKILLS, AND ABILITIES:

QUALIFICATIONS:

  • Education: Requires Master's Degree from an accredited school of SocialWork (MSW) or higher.

  • Requires knowledge of community resources and agencies in relation tohealth, illness, disability, and social determinants of health at the local, state,and national levels.
  • Demonstrates cultural humility and comfort with a diverse patient population.

The Clinician (MSW/MFT/PCC) is working as part of a multidisciplinary team servicing metro lines and stations and nearby encampments in the LA metro area to address the unhoused population. Clinicians will work on a team with outreach workers, medical staff, SUD counselors, and case managers in order to increase access to housing and services for unhoused riders. This program is in coordination with the LA County Department of Health Services (DHS). The right candidate should have a passion for helping the unhoused and want to work in a team environment.

Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity.

The target population of this office is individuals who are gravely disabled, (unable to provide for food, clothing and shelter as a result of chronic mental illness and/or chronic alcoholism), as a result of major mental illness as defined by the Diagnostic Statistical Manuel (DSM). The population also includes individuals who may have involvement with the Criminal Justice System and are being held in jail and State Hospitals or other facilities designated for those who have committed, or are alleged to have committed a crime.

The Office of Conservatorship Services has an average caseload of 700, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350.

The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders.

Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).

This is the entry-level class in the Protective Services Worker series. Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS. Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected “elder” and “dependent adult” abuse including neglect, self-neglect, and financial abuse. The PSW performs professional investigations and case management services to victims of abuse.
